Osteopathic Health of Saratoga is pleased to offer school and sport physicals. Please bring all forms you need completed to your visit.

We are out-of-network providers with all insurance companies, including Medicare.  Your health insurance is a contract between you and your insurance company only.  Full payment is due at time of service.  For your convenience, you can pay with a credit card, flex card, or personal check.  Overdrafted checks will incur a $35 fee.  You will be provided the necessary paperwork to submit to your insurance company for reimbursement.  Reimbursement rates differ according to your insurer and plan.  For more information on reimbursement for your particular insurance plan, it is best to contact your insurance company.

Please wear comfortable, loose fitting, lightweight clothing.  No jeans, skirts, or dresses please.  We are a fragrance free office because we care for many patients with asthma and allergies.  Please do not wear perfume, cologne, or anything with heavy scents on the day of your appointment.

Your appointment is time reserved specially for you and your physician.  We do not double-book at Osteopathic Health of Saratoga.  If you cannot make your appointment, we ask that you cancel your appointment at least 24 hours in advance by (calling or e-mailing).  Without a 24-hour notice, patients who forget or miss their appointments, cancel, or reschedule will incur a $75 fee.  Monday appointments must be cancelled by the previous Friday before 10:00 am.  Of course, exceptions are made for true emergencies.

Kindly print, read, and complete the New Patient Forms the best you can and bring them with you to your appointment.  Please arrive 20 minutes prior to your appointment to allow time to process your paperwork and complete the registration process.

Please bring the following to your initial visit:​

• A printed and completed copy of the New Patient Registration and Medical History Forms

• A printed and completed copy of the Consent Forms

• Health Insurance card and driver’s license (photo ID)

• Copies of your medical records, including lab and diagnostic tests (ex. blood work results and radiology reports) that pertain to the reason for your visit, such as X-rays, MRIs, CAT scans, etc., along with any other related studies such as NCS/EMGs

• A copy of your immunization records  

• A list of all the medications, vitamins, and supplements you currently take, including amounts and when you take them
